Ongoing support for your health
Well Care offers Personalized Care Management, a Medicare-sponsored program that gives you support beyond your regular doctor visits. Through this program, you’ll work with a dedicated nurse who checks in monthly to help you manage chronic conditions and stay on track with your health goals.

Who qualifies?
You may be eligible if you live with two or more qualifying conditions such as Alzheimer's, Arthritis, Asthma, Cancer, COPD, Depression, Diabetes, Heart Failure, Hypertension, and Osteoporosis.

Well Care is a Federally Qualified Health Center (FQHC). This means that we are here to help our community receive quality health care by removing obstacles that underserved groups sometimes experience. By providing sliding fee payment options, transportation assistance, telehealth, and translation services, we can help serve members of our community who may have neglected their health otherwise.
