Are you a new patient?
Save valuable time by pre-registering before your first visit.
During your initial appointment, please ensure you have the following:
- Proof of Household Income: Bring documentation such as a pay stub, child support, Social Security/Disability, or TANF.
- Picture ID: Don't forget to bring a valid driver's license or identification card.
- List of Medications: For your convenience, jot down your current prescription medications on the Patient History sheet.
- Insurance Information: Provide a copy of your insurance, Medicare, or Medicaid card. (Discounted fees are available for qualifying patients based on household size and income.)
For New Patients:
Pre-register online to streamline your check-in process, or feel free to fill out pre-registration forms in person during your visit. We're committed to making your first visit as smooth as possible.
Well Care Welcomes Walk-Ins

Life can be hectic, and finding time for medical appointments can be a challenge. That's why Well Care Community Health offers convenient walk-in visits designed to fit seamlessly into your busy schedule.
Whether it's back-to-school physicals, sports physicals, immunizations, minor illness, or any other medical need — take advantage of the convenience of Well Care’s Walk-Ins and put your health first!
Accepting walk-in visits Monday - Friday, 8 am - 4 pm. Everyone is welcome, regardless of insurance or ability to pay.
Walk-in for:
- Well child exams
- Sports physicals
- Chronic follow-up
- Fever
- Ear pain
- Sore throat
- Sinus Infections
- Vomiting
- Rash & sunburn
- Flu & cold symptoms
Dental and Behavioral Care, along with Annual Wellness for Medicare patients are not included.
