Diabetes Prevention Program

CDC National Diabetes Prevention Program – PreventT2!

The diabetes prevention program is a lifestyle modification program targeting nutrition, physical activity, and other stressors. These things contribute to blood sugar, weight gain, and ultimately an increased risk for diabetes, high blood pressure, and high cholesterol.

What you can expect
This year-long program meets weekly for 16 weeks, every 2 weeks for 4 sessions, and then monthly for 6 months. Classes are around one hour long and consist of a weigh-in, class time, and group discussion. Participants will keep an activity log, food log, and create an action plan for each class that will be reviewed by the lifestyle coach. Participants will meet with the lifestyle coach at least monthly throughout the program to go over individualized goals and progress.

Individualized counseling
Participants will meet at least monthly with a pharmacist or peer support to:

  • Discuss nutrition and physical activity
  • Create and manage goals to promote individual success
  • Assess lab work and progress
  • Medication review
  • Provide assistance and resources if barriers to care are identified
  • Answer questions and concerns regarding the program, nutrition, physical activity, or lifestyle modifications

Program schedule
New sessions begin every 3 months, rotating with a 10am and 5:30pm option. We want to make sure all of our participants have a time that fits their schedule.

Course materials and translation available for Spanish-speaking individuals.
