David Keller, MD

Dr. Keller obtained his M.D. degree from the University of Cincinnati College of Medicine. He has practiced medicine in the Richmond area for over 35 years. He lives with his wife, Pamela. In his spare time, he enjoys running, playing board games with friends and family, and watching videos of his granddaughter who is growing up fast in Florida.
William Cooke, MD, FAAFP, FASAM, AAHIVS

Dr. Cooke graduated with honors from IU School of Medicine in 2001 and is a Fellow of the American Academy of Family Medicine and the American Society of Addiction Medicine. He specializes in Family, Addiction, and HIV Medicine and has earned multiple honors, including Family Physician of the Year and the Physician Pillar of Excellence Award. Dr. Cooke serves as Executive Director of Refresh (F5), Medical Director at Wooded Glen Recovery Center, and Co-Director of the Rural Center for HIV/STD Prevention. He lives in Southern Indiana where he now resides with his wife and six kids.
Kaitlin Dunn, FNP

Kaitlin Dunn is an NCCPA board-certified Physician Assistant from southern Indiana. She graduated cum laude from Hanover College in 2020, and later received her Masters of Physician Assistant Studies in 2023 from Indiana University – Indianapolis. After living in Indianapolis for 3 years, she returned to southern Indiana to pursue her passion for rural community health. At Well Care — Foundations Family Medicine, Austin, she provides full-spectrum family medicine care, with an emphasis on adult chronic condition management, addiction medicine, HIV and hepatitis C management. She focuses on patient-centered care to create realistic and achievable goals for her patients, so that they can play an active role in their treatment plans. Outside of work, Kaitlin enjoys traveling, cooking and baking, and spending time with her family, friends, and dog.

Miranda Hernly, FNP-C, is an AANP board-certified Nurse Practitioner. She brings more than 11 years of nursing experience in acute care, home health, wound and ostomy care, and outpatient surgery. Miranda is passionate about providing high-quality, patient-centered primary care for individuals and families, caring for patients across the lifespan from infancy through adulthood. She values building long-term relationships with her patients and empowering them to take an active role in their health.

A Charlestown native and 2020 Charlestown High School graduate, she values family, friendship, and time spent with her loved ones—and her beloved pups.

With experience in emergency, long-term, addiction, and family medicine, Katie is passionate about preventive care and meeting patients where they are in their wellness journey. She enjoys family time, traveling, and music, and serves patients in Austin and the Healthy Eagle Clinic.

In her free time, she enjoys hiking with her husband and spending busy days with her three young boys.

Scarlett earned her Doctoral degree from the University of Kentucky in 2020. She is passionate about mission work through Northside Christian Church, serving in places like Israel, Brazil, Guatemala, Poland, and Africa. At home, she and her husband, Jimmy, a hospice nurse, stay busy with their three teenagers. Scarlett enjoys traveling, family time, and providing compassionate, whole-person care to her patients.

Nancy graduated from the University of Louisville with a Bachelor's and Master's Degree in nursing, with certification as a Family Nurse Practitioner. Nancy has worked in various healthcare settings throughout her career including medical offices, surgery, home health, hospice, dialysis and endoscopy. A native of Jefferson County, she and her family now reside in Scott County, where she is actively involved with her grandchildren. Nancy is an advocate for preventative care and patients taking a proactive role in their health.

Michelle Thompson, MSN, APRN, FNP-BC, is a certified Family Nurse Practitioner who earned her master’s degree from Spalding University in 2021. She began her nursing career in 2013 after completing degrees at Ivy Tech Community College and Indiana Wesleyan. Her background includes oncology, critical care, and long-term care, with additional experience treating hepatitis C. Michelle specializes in chronic disease management, addiction medicine, hepatitis C treatment, and HIV care. Outside of work, she enjoys family time, home projects, and watching sports.

With eight years of experience as an emergency department nurse, Yessica cares for patients of all ages. Bilingual in Spanish, she enjoys supporting Spanish-speaking patients. Oscar Valencia, PharmD, serves as the Pharmacist in Charge at Well Care Community Health RX in Austin, Indiana. A proud Scottsburg native and 2012 graduate of Scottsburg High School, Dr. Valencia earned his Doctor of Pharmacy degree from Purdue University in 2019. Following graduation, Dr. Valencia completed a PGY-1 Pharmacy Residency at University Medical Center of El Paso in Texas. He then worked as a Clinical Critical Care Pharmacist at The Hospitals of Providence - East Campus in El Paso, where he developed expertise in caring for critically ill patients until returning to southern Indiana in 2025. Dr. Valencia is passionate about serving his home community and ensuring patients have access to quality pharmacy care. He brings both clinical expertise and a deep commitment to the Well Care mission of removing barriers to exceptional healthcare. Outside of the pharmacy, Dr. Valencia enjoys spending time with his wife, Paola, and their two children, Emilio and Amelia. His interests include traveling with family, running outdoors, and watching movies.
